To schedule an appointment, please call:
Dr. Fazio is also on call. If there is an urgent need to reach the doctor outside of normal office hours, please call the office and the doctor will be notified immediately.
Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule.

Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient.
Emergencies
Our office provides on-call service for emergencies, 24 hours-a-day, 7 days-a--week. Call and Dr. Fazio will be contacted immediately.
Insurance & Payment
We will submit forms for 3rd party insurance payments as partial payments only. Our office accepts care-credit no interest loans for dental use only. For your convenience, we also accept the following credit cards:
